Sefer Chareidim by Rabeinu Elazar Azkari ספר חרדים לרבינו אלעזר אזקרי
Sefer Chareidim is a very famous work on the 613 Mitzvos, by R' Elazar Azkari, who lived in the city of Tzfas at the end of the 16th century and was a comtemporary of the Arizal and the Beis Yosef. It is very unique in its style for a couple of reasons. First, all mitzvos are aranged according to the human body parts and time on which they depend in their observance. Second, Sefer Chareidim does not just have a single count of mitzvos according to his own opinion like all of the preceeding Mitzvos seforim do, but rather lists mitzvos according to many opinions of the Rishonim.
